From 1 July 2025, Medicare is introducing major updates to the way GP’s approach Chronic Disease Management (CDM), replacing care plans with GPCCMP, designed to simplify processes and improve continuity of care for patients with long-term conditions.
What’s Changing?
The current GP Management Plans (GPMPs) and Team Care Arrangements (TCAs) will be replaced by a single streamlined plan: the GP Chronic Condition Management Plan (GPCCMP).
This new format:
- Reduces administrative complexity
- Removes the requirement to involve two collaborating providers
- Allows direct referral to allied health by the GP
Updated MBS Items
From 1 July, GPs will bill the following new item numbers:

Key Points about GPCCMP
- Review Intervals:
- Plans can be reviewed every 3 months as clinically required – no need to prepare a new plan every year.
- Plans will be considered out of date if they were not written or reviewed within the previous 18 months; other than that, there is no time limit.
- Eligibility:
- Patients must have a chronic condition that has lasted or expected to last 6 months or more, or a terminal illness. GPs use clinical discretion—no fixed list of eligible conditions. (The definition of a chronic condition is not changing)
- MyMedicare Requirement:
- Patients enrolled in MyMedicare must receive services through their registered practice.Unregistered patients can continue accessing CDM through their usual GP.
- Referrals:
- GPs can now refer patients directly to allied health professionals without needing to involve two or more collaborators. Referrals will be valid for 18 months, unless otherwise specified
Transition Period
- Existing GPMPs/TCAs can continue to be used until 30 June 2027.
- After this, only GPCCMPs will be valid for:
- Allied health referrals
- Domiciliary medication management reviews (HMRs, RMMRs)
